Notice Regarding Form 1095

Form 1095 is now available and may be accessed and downloaded at any time through Employee Connect. This notice satisfies the requirement to inform employees that the form is available upon request. If a request is made, the form will be provided no later than January 31, 2026, or within 30 days of the request, whichever is later. 

Employees may access and download their Form 1095, W-2s, and pay stubs using the Employee Connect website below:

Login Information

  • Username: First three letters of your last name (ALL CAPS) + last four digits of your Social Security number
  • Password:
  • First letter of your first name
  • First letter of your last name
  • Last four digits of your Social Security number
  • @
  • Your year of birth

Example:
Username: NORxxxx
Password: pnxxxx@19xx
